Prepare for my first visit

HOW TO PREPARE

  1. Read and complete the New Patient Registration Packet.
  2. If you are or have been examined or treated for your pain by another physician, please fill out and send us a Record Request and Release Form for each physician or hospital.
  3. Confirm the address and travel directions to your appointment.
  4. Make a list of anything you want to tell your physician about your pain, and questions you want to remember to ask.

Note that all of our offices are designed to be fully accessible for all our patients. However, if you have any concerns or accommodations you would like to discuss, please do contact the office where you will have your appointment. We want your visit to go as smoothly for you as possible.

WHAT TO BRING

  1. All of your insurance cards and your photo ID.
  2. Results of any MRI, X-Ray, CT, EMG or other diagnostic tests related to your condition.
  3. Relevant medical records, especially those from another pain management physician.
  4. All pain medications you are currently taking.*

*Some prescription medications may only be prescribed by one physician. For your health, safety, and effective care, it’s important that we know all medicines you are taking and which physicians have prescribed them.

WHAT TO EXPECT

Where are you hurting? How badly? All the time, or does it come and go? What seems to help and what doesn’t? How is this impacting your life?

During your first visit, your Alliance physician and nurses want to understand your pain and your experience of it. They’ll review your medical history and ask you about your pain.

Your physician will conduct a thorough medical examination of you and attempt to identify the cause of your pain. Then they’ll discuss with you a comprehensive treatment plan to bring you real relief.

If you’re nervous about your first visit, that’s natural. Please tell us about anything that’s concerning you, and ask us any questions you have. When we better understand your experience, we can design a more effective plan for your relief.
